Bilingual study notes on cell death and necrosis: each point is given in English with Gujarati alongside.

1. Two kinds of cell death – કોષ મૃત્યુના બે પ્રકાર

Cells die in two main ways: necrosis and apoptosis. They differ in mechanism, appearance and role in the body. કોષ મુખ્યત્વે બે રીતે મરે છે: નેક્રોસિસ અને એપોપ્ટોસિસ. બંનેની પદ્ધતિ, દેખાવ અને શરીરમાં ભૂમિકા જુદી છે.
Necrosis is "accidental" death. The injury is so severe that too many parts of the cell are damaged and it breaks down. નેક્રોસિસ "આકસ્મિક" મૃત્યુ છે. ઈજા એટલી તીવ્ર હોય છે કે કોષના ઘણા ભાગ બગડી જાય છે અને કોષ વિખરાઈ જાય છે.
It goes with severe mitochondrial damage, loss of ATP, and rupture of lysosomal and plasma membranes. તેમાં મિટોકોન્ડ્રિયાને ગંભીર નુકસાન, ATP ની ઘટ, અને લાયસોસોમ તથા પ્લાઝ્મા મેમ્બ્રેનનું ફાટવું જોવા મળે છે.
If reversible injury continues because the cause is not removed, it ends in necrosis. કારણ દૂર ન થાય અને પરિવર્તનીય ઈજા ચાલુ રહે, તો તેનો અંત નેક્રોસિસમાં આવે છે.
Necrosis always brings local inflammation, which clears the dead cells. નેક્રોસિસ સાથે હંમેશાં સ્થાનિક દાહ થાય છે, જે મૃત કોષોને સાફ કરે છે.
Apoptosis is "regulated" death. It runs through defined molecular pathways and removes cells cleanly, without inflammation or damage to neighbours. એપોપ્ટોસિસ "નિયંત્રિત" મૃત્યુ છે. તે ચોક્કસ આણ્વિક માર્ગોથી થાય છે અને દાહ કે આસપાસના નુકસાન વિના કોષને દૂર કરે છે.
The line between the two is not absolute: necroptosis is a genetically controlled form of necrosis, some cells show features of both, and apoptosis can pass into necrosis. બંને વચ્ચેની રેખા સંપૂર્ણ નથી: નેક્રોપ્ટોસિસ જનીનીય રીતે નિયંત્રિત નેક્રોસિસ છે, કેટલાક કોષોમાં બંનેનાં લક્ષણો દેખાય છે, અને એપોપ્ટોસિસ નેક્રોસિસમાં ફેરવાઈ શકે છે.

2. Necrosis: causes – નેક્રોસિસનાં કારણો

3. What happens in a necrotic cell – નેક્રોટિક કોષમાં શું થાય છે

Cell proteins are denatured. કોષના પ્રોટીન વિકૃત (denature) થાય છે.
Lysosomal membranes break; their enzymes enter the cytoplasm and digest the cell. લાયસોસોમનું પટલ તૂટે છે; તેના ઉત્સેચકો સાયટોપ્લાઝમમાં આવીને કોષને પચાવે છે.
The plasma membrane breaks; cell contents leak into the surrounding tissue. પ્લાઝ્મા મેમ્બ્રેન તૂટે છે; કોષના ઘટકો આસપાસની પેશીમાં નીકળે છે.
The leaked contents provoke inflammation. બહાર નીકળેલા ઘટકો દાહ પ્રેરે છે.
Leukocytes arrive, release more enzymes and phagocytose the debris, so the dead cells are cleared. શ્વેતકણો આવે છે, વધુ ઉત્સેચકો છોડે છે અને કચરો ગળી જાય છે, તેથી મૃત કોષો સાફ થાય છે.

DAMPs (damage-associated molecular patterns)

DAMPs are molecules that normally stay inside healthy cells. Their release signals severe injury. DAMPs એવા અણુ છે જે સામાન્ય રીતે સ્વસ્થ કોષની અંદર રહે છે. તે બહાર આવે એ ગંભીર ઈજાનો સંકેત છે.
Examples: ATP from damaged mitochondria, and uric acid from DNA breakdown. ઉદાહરણ: નુકસાન પામેલા મિટોકોન્ડ્રિયામાંથી ATP, અને DNA ના વિઘટનથી બનતો યુરિક એસિડ.
Receptors on macrophages and other cells recognise DAMPs, leading to phagocytosis and cytokine production. મેક્રોફેજ અને અન્ય કોષોના રિસેપ્ટર DAMPs ને ઓળખે છે, જેથી ફેગોસાયટોસિસ અને સાયટોકાઇનનું ઉત્પાદન થાય છે.

4. Why necrosis shows up in blood tests – લોહીની તપાસમાં નેક્રોસિસ

Proteins leak out through the damaged membrane and reach the blood. Each tissue has its own marker.

Cardiac troponin can be found in blood from about 2 hours after heart muscle necrosis, well before the infarct is visible under the microscope. હૃદય સ્નાયુના નેક્રોસિસ પછી લગભગ 2 કલાકમાં કાર્ડિયાક ટ્રોપોનિન લોહીમાં મળી શકે છે, એટલે કે માઇક્રોસ્કોપમાં ઇન્ફાર્ક્ટ દેખાય તેના ઘણા સમય પહેલાં.
Repeated troponin measurement is therefore central to diagnosing and managing myocardial infarction. તેથી મ્યોકાર્ડિયલ ઇન્ફાર્ક્શનના નિદાન અને સારવારમાં ટ્રોપોનિનનું વારંવાર માપન મુખ્ય સ્થાન ધરાવે છે.

5. The point of no return – પાછા ન ફરી શકાય તેવું બિંદુ

The exact moment when injury becomes irreversible is not defined. ઈજા ક્યારે અપરિવર્તનીય બને છે તે ચોક્કસ ક્ષણ નક્કી નથી.
Two features mark irreversibility: બે લક્ષણો અપરિવર્તનીયતા દર્શાવે છે:
1. Mitochondria cannot recover: no oxidative phosphorylation and no ATP, even after the cause is removed. 1. મિટોકોન્ડ્રિયા સાજા થતા નથી: કારણ દૂર થયા પછી પણ ઓક્સિડેટિવ ફોસ્ફોરાયલેશન અને ATP બનતું નથી.
2. Membrane function is severely disturbed. 2. પટલનું કાર્ય ગંભીર રીતે ખોરવાય છે.
Knowing this point matters clinically, because treatment given before it can save the tissue. આ બિંદુ જાણવું ક્લિનિકલી મહત્વનું છે, કારણ કે તેના પહેલાં અપાયેલી સારવાર પેશીને બચાવી શકે છે.

6. Morphology of necrosis – નેક્રોસિસનું આકારવિજ્ઞાન

More eosinophilic (pink) on H&E: RNA is lost, and denatured proteins bind eosin. H&E માં વધુ ઇઓસિનોફિલિક (ગુલાબી): RNA ઘટે છે, અને વિકૃત પ્રોટીન ઇઓસિન સાથે જોડાય છે.
Glassy, homogeneous look, because glycogen is lost. કાચ જેવો, એકસરખો દેખાવ, કારણ કે ગ્લાયકોજન જતું રહે છે.

Pyknosis – પિક્નોસિસ Nucleus shrinks and becomes dense and dark. Also seen in apoptosis. કેન્દ્રક સંકોચાઈને ઘટ્ટ અને ઘેરું બને છે. એપોપ્ટોસિસમાં પણ જોવા મળે છે.
Karyorrhexis – કેરિયોરેક્સિસ The pyknotic nucleus breaks into fragments. પિક્નોટિક કેન્દ્રક ટુકડાઓમાં તૂટે છે.
Karyolysis – કેરિયોલાયસિસ The blue staining fades as endonucleases digest the DNA. એન્ડોન્યુક્લિએઝ DNA ને પચાવે તેમ વાદળી રંગ ઝાંખો પડે છે.

In 1 to 2 days the nucleus disappears completely. – 1 થી 2 દિવસમાં કેન્દ્રક સંપૂર્ણ અદૃશ્ય થઈ જાય છે.

7. Patterns of tissue necrosis – પેશી નેક્રોસિસના પ્રકાર

The pattern gives a clue to the cause. – પ્રકાર પરથી કારણનો સંકેત મળે છે.

Coagulative – કોએગ્યુલેટિવ Tissue outline is preserved for days; tissue is firm. Enzymes are denatured too, so the dead cells are not digested at once. Seen in infarcts of all organs except the brain. પેશીની રૂપરેખા દિવસો સુધી જળવાય છે; પેશી કઠણ હોય છે. ઉત્સેચકો પણ વિકૃત થાય છે, તેથી મૃત કોષો તરત પચતા નથી. મગજ સિવાયનાં બધાં અંગોના ઇન્ફાર્ક્ટમાં જોવા મળે છે.
Liquefactive – લિક્વિફેક્ટિવ Dead cells are digested into a thick liquid. Seen in bacterial and some fungal infections (pus) and in hypoxic death in the brain. મૃત કોષો પચીને ઘટ્ટ પ્રવાહી બને છે. બેક્ટેરિયલ અને કેટલાક ફૂગના ચેપમાં (પરુ) તથા મગજમાં હાઇપોક્સિક મૃત્યુમાં જોવા મળે છે.
Gangrenous – ગેંગ્રીનસ A clinical term, usually for a limb (often the lower leg) that has lost its blood supply: coagulative necrosis of several tissue layers. Added bacterial infection makes it liquefactive: wet gangrene. ક્લિનિકલ શબ્દ, સામાન્ય રીતે રક્ત પુરવઠો ગુમાવેલા અંગ (મોટે ભાગે પગ) માટે: અનેક સ્તરોનું કોએગ્યુલેટિવ નેક્રોસિસ. બેક્ટેરિયલ ચેપ ઉમેરાય તો લિક્વિફેક્ટિવ બને: વેટ ગેંગ્રીન.
Caseous – કેસિયસ Typical of tuberculosis. Crumbly, white, cheese-like. Microscopy: structureless granular debris surrounded by epithelioid macrophages, forming a granuloma. ટ્યુબરક્યુલોસિસની લાક્ષણિકતા. ભૂકો થાય તેવું, સફેદ, ચીઝ જેવું. માઇક્રોસ્કોપમાં: રચના વિનાનો દાણાદાર કચરો, આસપાસ એપિથેલિઓઇડ મેક્રોફેજ, જે ગ્રેન્યુલોમા બનાવે છે.
Fat – ફેટ Seen in acute pancreatitis. Pancreatic lipases split fat; the fatty acids combine with calcium to give chalky white spots (saponification). Microscopy: shadowy outlines of fat cells, blue calcium deposits, inflammation. એક્યુટ પેન્ક્રિયાટાઇટિસમાં જોવા મળે છે. સ્વાદુપિંડના લાયપેઝ ચરબી તોડે છે; ફેટી એસિડ કેલ્શિયમ સાથે જોડાઈને ચોક જેવા સફેદ ડાઘ બનાવે છે (સેપોનિફિકેશન). માઇક્રોસ્કોપમાં: ચરબી કોષોની ઝાંખી રૂપરેખા, વાદળી કેલ્શિયમ જમાવ, દાહ.
Fibrinoid – ફાઇબ્રિનોઇડ Damage to vessel walls in immune reactions. Immune complexes and leaked plasma proteins give a bright pink, structureless look on H&E. રોગપ્રતિકારક પ્રતિક્રિયામાં રક્તવાહિનીની દીવાલને નુકસાન. ઇમ્યુન કોમ્પ્લેક્સ અને લીક થયેલા પ્લાઝ્મા પ્રોટીન H&E માં તેજસ્વી ગુલાબી, રચના વિનાનો દેખાવ આપે છે.

An infarct is a localised area of coagulative necrosis. – ઇન્ફાર્ક્ટ એટલે કોએગ્યુલેટિવ નેક્રોસિસનો સ્થાનિક વિસ્તાર.

Gross: a pale, firm, wedge-shaped infarct with a red border. Microscopy: tubules and glomeruli keep their outline but are deep pink and have lost their nuclei. – ગ્રોસ: ફિક્કો, કઠણ, ફાચર આકારનો ઇન્ફાર્ક્ટ, લાલ કિનારી સાથે. માઇક્રોસ્કોપમાં: નલિકાઓ અને ગ્લોમેરુલાઇની રૂપરેખા જળવાય છે, પણ તે ઘેરા ગુલાબી છે અને કેન્દ્રક ગુમાવી ચૂક્યા છે.

Caseous: cheese-like centre inside a granuloma (tuberculosis). Liquefactive: tissue turned to liquid (brain infarct, abscess). Fibrinoid: bright pink material in a vessel wall. – કેસિયસ: ગ્રેન્યુલોમાની અંદર ચીઝ જેવું કેન્દ્ર (ટ્યુબરક્યુલોસિસ). લિક્વિફેક્ટિવ: પેશી પ્રવાહી બની જાય (મગજનો ઇન્ફાર્ક્ટ, એબ્સેસ). ફાઇબ્રિનોઇડ: રક્તવાહિનીની દીવાલમાં તેજસ્વી ગુલાબી પદાર્થ.

8. What happens to necrotic tissue – નેક્રોટિક પેશીનું પછી શું થાય છે

Most dead cells are removed by enzyme digestion and phagocytosis by leukocytes. મોટા ભાગના મૃત કોષો ઉત્સેચકીય પાચન અને શ્વેતકણો દ્વારા ફેગોસાયટોસિસથી દૂર થાય છે.
If debris is not removed quickly, calcium salts deposit in it. This is dystrophic calcification. કચરો ઝડપથી દૂર ન થાય તો તેમાં કેલ્શિયમના ક્ષાર જમા થાય છે. આ ડિસ્ટ્રોફિક કેલ્સિફિકેશન છે.

Six patterns: coagulative, liquefactive, gangrenous, caseous, fat, fibrinoid. છ પ્રકાર: કોએગ્યુલેટિવ, લિક્વિફેક્ટિવ, ગેંગ્રીનસ, કેસિયસ, ફેટ, ફાઇબ્રિનોઇડ.
