Cellular Adaptations: Hypertrophy, Hyperplasia and Atrophy
When their environment changes, cells adapt by changing their size, number, phenotype, metabolic activity or function. These changes are reversible: if the stimulus is removed, the cell can return to normal. Medical terms are kept in English inside the Gujarati text, as they are usually taught.

1. Hypertrophy |
Definition |
| English |
|---|---|
| Hypertrophy is an increase in the size of cells, which leads to an increase in the size of the organ. | |
| A hypertrophied organ has no new cells, only bigger cells. The cells grow because they make and assemble more internal structural components. |
| Cells that can divide may respond to stress with both hyperplasia and hypertrophy. Cells that cannot divide (e.g., myocardial fibres) can only enlarge, so tissue mass increases by hypertrophy alone. |
| In many sites, hypertrophy and hyperplasia occur together, and both enlarge the organ. |
| Hypertrophy can be physiologic or pathologic. It is caused by increased functional demand or by stimulation from hormones and growth factors. |
Pathologic hypertrophy |
| English |
|---|---|
| Striated muscle cells of the heart and skeletal muscle have limited capacity to divide. They meet increased metabolic demand mainly by hypertrophy. |
| The commonest stimulus is increased workload. The muscle cells make more protein and more myofilaments per cell, so each myocyte generates more force, and the whole muscle becomes stronger. |
| Classic example: enlargement of the heart in response to pressure overload, usually from hypertension or valvular disease. |
| At first, cardiac hypertrophy improves function. Over time the adaptation often fails, setting the stage for heart failure and other forms of heart disease. |

Physiologic hypertrophy |
| English |
|---|---|
| Uterus in pregnancy: a good example of hormone-induced organ enlargement, mainly due to hypertrophy of smooth muscle fibres. |
| Oestrogen acts through oestrogen receptors, which increases synthesis of smooth muscle proteins and enlarges the cells. |
| Bodybuilders: the bulging muscles of people "pumping iron" result from enlargement of individual skeletal muscle fibres in response to increased demand. |

Mechanisms of hypertrophy |
| English |
|---|---|
| Hypertrophy results from increased production of cellular proteins. Most of what we know comes from studies of the heart, because beyond a certain point myocardial hypertrophy becomes maladaptive. |
| It is driven by growth factors and by the direct effect of mechanical force on pathways that stimulate protein synthesis. |
| Step 1: mechanical sensors in the cell detect the increased load. |
| Step 2: the sensors activate signalling pathways. The PI3K/AKT pathway is most important in physiologic hypertrophy (e.g., exercise). G-protein-coupled receptor pathways, activated by growth factors and vasoactive agents, are more important in pathologic hypertrophy. |
| Step 3: signalling also increases production of growth factors (TGF-β, IGF-1, fibroblast growth factor) and vasoactive agents (α-adrenergic agonists, endothelin-1, angiotensin II). |
| Step 4: these pathways activate transcription factors GATA4, NFAT (nuclear factor of activated T cells) and MEF2 (myocyte enhancer factor 2), which switch on genes for contractile proteins. | પગલું 4: આ માર્ગો transcription factors GATA4, NFAT (nuclear factor of activated T cells) અને MEF2 (myocyte enhancer factor 2) ને સક્રિય કરે છે, જે contractile proteins ના genes ચાલુ કરે છે. |
| Fetal gene switch: the heart switches from adult to fetal isoforms of contractile proteins. For example, the α isoform of myosin heavy chain is replaced by the β isoform, which contracts more slowly but more economically. |
| ANF: hypertrophy increases expression of atrial natriuretic factor, a hormone that makes the kidney excrete salt, lowers blood volume and pressure, and so reduces the haemodynamic load. |

When adaptation fails |
| English |
|---|---|
| Whatever its cause, cardiac hypertrophy eventually reaches a limit beyond which more muscle mass cannot cope with the load. |
| Myocardial fibres then show regressive changes, the most important being degeneration and loss of myofibrillar contractile elements. In extreme cases, myocytes die. |
| The net result is heart failure. This shows how an adaptation to stress can progress to significant cell injury if the stress is not relieved. |

2. Hyperplasia |
Definition |
| English |
|---|---|
| Hyperplasia is an increase in the number of cells in an organ or tissue in response to a stimulus. |
| Hyperplasia and hypertrophy are distinct processes, but they often occur together and can be triggered by the same stimulus. |
| Hyperplasia can happen only if the tissue contains cells capable of dividing. It can be physiologic or pathologic. |
Physiologic hyperplasia |
| English |
|---|---|
| Caused by hormones or growth factors, when the functional capacity of hormone-sensitive organs must increase, or when compensatory growth is needed after damage or resection. |
| Hormonal hyperplasia: proliferation of the glandular epithelium of the female breast at puberty and in pregnancy, usually along with hypertrophy of the epithelial cells. |
| Compensatory hyperplasia — liver: in people who donate a lobe of liver for transplant, the remaining cells divide so fast that the liver soon regains its original size. Experimental partial hepatectomy has been very useful for studying liver regeneration. |
| Bone marrow: after acute bleeding or premature destruction of red cells (haemolysis), feedback loops involving erythropoietin stimulate red cell production, which can rise up to eight-fold. |

Pathologic hyperplasia |
| English |
|---|---|
| Most forms are caused by excessive or inappropriate action of hormones or growth factors on target cells. |
| Endometrial hyperplasia: normally, after menstruation, pituitary hormones and ovarian oestrogen stimulate rapid endometrial proliferation. Rising progesterone, usually 10–14 days before the end of the cycle, stops it. |
| If the oestrogen–progesterone balance is disturbed and oestrogen is relatively high, the endometrial glands become hyperplastic. This is a common cause of abnormal uterine bleeding. |
| Benign prostatic hyperplasia (BPH) is another common example, caused by hormonal stimulation by androgens. |
| In these forms the process remains controlled: if the hormonal stimulus is removed, the hyperplasia can regress or stop. |
| Link to cancer: more cell division means more chance of acquiring genetic errors. Hyperplasia is not cancer, but pathologic hyperplasia is fertile soil in which cancer may later arise. For example, patients with endometrial hyperplasia have a higher risk of endometrial cancer. |
| Viral infection: hyperplasia is a characteristic response to some viruses, such as papillomaviruses, which cause skin warts and lesions made of masses of hyperplastic epithelium. The virus makes factors that interfere with host proteins controlling cell proliferation. Some of these virus-induced proliferations are also precursors of cancer. |

Mechanisms of hyperplasia |
| English |
|---|---|
| Hyperplasia results from growth factor–driven proliferation of mature cells and, in some cases, from increased output of new cells from tissue stem cells. |
| After partial hepatectomy, the liver produces growth factors that bind receptors on the surviving hepatocytes and activate signalling pathways that stimulate cell proliferation. |
| If hepatocytes cannot proliferate, as in some forms of hepatitis that damage them, the liver can regenerate from intrahepatic stem cells instead. |
3. Atrophy |
Definition |
| English |
|---|---|
| Atrophy is a reduction in the size of an organ or tissue due to a decrease in cell size and number. | Atrophy |
| Physiologic atrophy is common during normal development. Some embryonic structures, such as the notochord and thyroglossal duct, undergo atrophy during fetal development. |
| Pathologic atrophy has many causes and can be local or generalised. |
Causes of atrophy |
| Cause / કારણ | English | ગુજરાતી |
|---|---|---|
| Decreased workload (disuse atrophy) | A fractured bone in a plaster cast, or complete bed rest, leads to rapid skeletal muscle atrophy. The early shrinkage reverses once activity resumes. With longer disuse, fibre number also falls (by apoptosis), and increased bone resorption can cause disuse osteoporosis. |
| Loss of innervation (denervation atrophy) | Normal metabolism and function of skeletal muscle depend on its nerve supply. Nerve damage leads to atrophy of the fibres it supplies. |
| Diminished blood supply | Slowly developing arterial occlusive disease causes gradual reduction in blood supply (chronic ischaemia) and tissue atrophy. In late adult life, the brain may shrink progressively, mainly from atherosclerosis: senile atrophy. |
| Inadequate nutrition | In severe protein–calorie malnutrition (marasmus), skeletal muscle protein is used for energy after fat stores are exhausted, causing muscle wasting (cachexia). Cachexia is also seen in chronic inflammatory disease and cancer, where chronic overproduction of TNF is thought to suppress appetite and deplete lipids. |
| Loss of endocrine stimulation | Hormone-responsive tissues such as the breast and reproductive organs depend on hormones. Loss of oestrogen after menopause causes atrophy of the endometrium, vaginal epithelium and breast. The prostate atrophies after chemical or surgical castration (e.g., for prostate cancer). |
| Pressure | Prolonged compression can cause atrophy. An enlarging benign tumour can cause atrophy of the surrounding tissue, probably through ischaemia from pressure on its blood supply. |

Cellular changes in atrophy |
| English |
|---|---|
| The basic changes are the same in every setting. The first response is a reduction in cell size and organelles, which lowers the cell's metabolic needs enough for it to survive. |
| Atrophic muscle cells have fewer mitochondria and myofilaments and less rough ER. A new balance is reached between metabolic demand and the lower supply of blood, nutrition or trophic stimulation. |
| Early on, function falls but cell death is minimal. Atrophy from a slowly falling blood supply can progress until cells are irreversibly injured and die, often by apoptosis. Apoptosis also contributes to atrophy of endocrine organs after hormone withdrawal. |
Mechanisms of atrophy |
| English |
|---|---|
| Atrophy results from decreased protein synthesis and increased protein degradation. | Atrophy |
| Less synthesis: trophic signals (e.g., from growth factor receptors) normally increase nutrient uptake and mRNA translation. When these signals fall, protein synthesis falls. |
| More degradation: mainly by the ubiquitin–proteasome pathway. Nutrient deficiency and disuse activate ubiquitin ligases, which tag cellular proteins with the small peptide ubiquitin and send them to proteasomes for degradation. This pathway is also thought to cause the rapid proteolysis of catabolic states, including cancer cachexia. |
| Autophagy: in many situations atrophy is accompanied by increased autophagy, seen as more autophagic vacuoles. |
| Some debris inside autophagic vacuoles resists digestion and persists as membrane-bound residual bodies. An example is lipofuscin granules. In large amounts they give the tissue a brown colour: brown atrophy. |
